In accordance with IRS guidelines, LACASA is not automatically mailing 1095-C forms to employees this year. However, if you would like to receive a copy of your 2024 Form 1095-C, you may request one using the contact information below.

You may request a copy of your 2024 Form 1095-C by contacting us through one of the following methods:
